I know there is a Pinned thread in this forum and I posted this there a few days ago but nobody responded to it yet...so i am posting it here.

So from what I have read here, as long as my fiancee has had her medical exam within 1 year of sending in AOS papers then she doesn't need to get her 3025 transcribed to the I-693? Is that correct? She had her full medical in the Philippines along with her vaccinations. Can somebody please tell me if we need to send in the I-693. I called a civil surgeon in our area, Los Angeles, and the secretary was saying it would cost $175 to do a full medical exam again. I told her my fiancee already did that and we just need the papers transcribed and she said it doesn't matter, we need to do the entire process! That seems ridiculous if she has already done the medical.

One other question...Is there multiple shots she needs to get for MMR, tDap, and Hepatitis B? She got the vaccines in the Philippines but on the D-3025 the box "Insufficient Time Interval" is checked.

It also says "Applicant may be eligible for blanket waiver(s) because vaccinations not medically appropriate" What does that mean?

To answer your question. You need to transcribed her DS-3025 and bring it to a USCIS approved civil surgeon so they can fill up her I-693. She dont have to do the whole medical exam though if she entered US within a year following her medical exam. Yes a lot of civil surgeon will try to rip you off and insist on full medical exam but do some research and call in there is a thread here titled civil surgeons that don't ripped you off in the AOS family based forum for sure there is one near your area that will charge reasonable $ just for transcribing. I had done mine in Santa Ana and they only charge me $ 30 for transcribing got my sealed enveloped after 3 days.

You will get the shots in the health center near where you live for free. Visit the health clinic near where you live and bring her DS-3025 and ask them what shots does she still needs by showing them her DS-3025, if she needs anything they will give her for free and give you a yellow card that will list down the new shots they have given her. Take both the DS-3025 and the yellow card to the USCIS civil surgeon and have it transcribed.

Some people got away just by sending the DS-3025 and some got an RFE for not sending I-693 for the vaccinations.

They usually require two shots for the MMR, and the second shot can be given a month after the first. So it is likely she will need that.

The "Applicant may be eligible for blanket waiver(s) because vaccinations not medically appropriate" probably refers to vaccinations that are required for children and/or the elderly.

Shots can be not medically appropriate because of age, pregnant, allergic...the instructions to panel physicians details all that.
