What other shots are there that you haven't had? Remember that some of them are only required for children and/or the elderly.

It is up to you whether you get the shots done before visiting the civil surgeon or if you want the civil surgeon to administer the shots himself. Just make sure that if you get them done before you have proof of that.

Do you have the DS-3025 form? (Vaccination List) What does it say at the bottom? What box did they check?

Because you opted not to get your shots at St. Luke's I am concerned you will not have enough time to complete the other shots in series.

Ideally, you should have gotten all of your shots done at St. Luke's, since it's included in the fee you paid. If the DS-3025 says your shots are incomplete, you have to find out if you need booster shots or additional vaccines. Better to get them done through your personal physician (IF you are already insured) or through the local health department.

They will determine if you need boosters or vaccines based on your age and gender. If you can get shots from your personal physician or the local health department, you can show that proof to the Civil Surgeon so they do not charge you extra.

What you just need is to have your vaccinations transcribed on the I-1693 form...not get a whole new medical exam.

You have to go to a USCIS designated Civil surgeon. HE will tell you or give you the shots you need and give you a I-693 which you have to submit with the AOS application.

Again, you have to see your personal physician or local health center so they can tell you if you need those shots or not --- based on your age and gender. From what I know, polio vaccines are administered when you are a baby...if you have gone past the age requirement for polio vaccine they will not give it to you anymore because of your age.

You sound like one of the other members I replied to before...with a different ID.

The CS determines which shots you need. NOT your local doctor or health dep!!!
