I-485, vaccination supplement for a kid (12 y.o.)

When my fiancee passed the medical exam in Kyiv, they gave her a separate filled-out DS-3025 form (in addition to the sealed envelope) - but they said her son fine and would not need any additional vaccination records, so currently he does not any.

Now I am about to file the application, and I am wondering whether they done it the way they should in Kyiv. In I-485 instructions it says "you may include vaccination supplement" - so does it mean I do not have to file it for my step-son, since supposedly they have his vaccination info in his medical exam file? Or should I rather find a civil surgeon here in Seattle an get him that record?

Your step son needs to file a separate I-485. He pays a separate fee, $600 if under age 14 and filing with his mother, $101 if age 14 or over or filing separately from his mother. He will need the medical exam (which was in his secret envelope you turned in at the POE) and a vaccination supplement DS-3025 OR he will need the vaccinations and the I-693 filled out by a civil surgeon here in the US. Vaccinations are age specific, but even if he is younger than the age required, he will need an I-693 filled out by a CS saying the vaccines are "not age appropriate"

Well, this is exactly my question - his medical exam and vaccination records are in the brown envelope we turned over to the immigration officer and I do not have any additional paperwork on hand. Should I just file I-485, or should I find a CS and make him transcribe his vaccinations into I-693 from the medical record file we have?

Do this one. You must send in the transcrived I-693. That's what we did and no problems.

Do this one. You must send in the transcrived I-693. That's what we did and no problems.

Thanks, this is what I figured (better be safe than sorry). Already made an appointment with CS here here in Seattle (she's originally from Russia, so she should be able to decrypt the writing in Artem's medical records :) ).
