HELLO, WE ARE CURRENTLY TRYING TO GATHER EVERYTHING FOR THE AOS APPLICATION. IT'S UNDER A VISA K-1 AND I FOUND THIS ON THE INSTRUCTION SHEET FOR FORM I-485 AOS, IT SAYS

8.FIANCE

IF YOU ARE A K-1 FIANCE OR K-2 DEPENDENT WHO HAD A MEDICAL EXAMINATION WITHIN THE PAST YEAR AS REQUIRED FOR THE NONIMMIGRANT FIANCE VISA, YOU ONLY NEED TO SUBMIT A VACCINATION SUPPLEMENT, NOT THE ENTIRE MEDICAL REPORT. YOU MAY INCLUDE THE VACCIONATION SUPPLEMENT WITH YOUR FORM I-485.

MY QUESTION IS...HOW DO I DO THIS? DO I JUST MAKE A COPY OF HIS SHOTS AND SUBMIT IT ALONG WITH THE FORM I-485 OR IS THERE ANOTHER FORM TO FILL OURT REGARDING SHOTS??

They're asking for the DS-3025, which is the list of vaccinations your spouse received back in the home country...do you have it?

02-23-2018: Date on NOA, retrieved from online account

02-23-2018: Date on Biometrics Appointment Letter (Biometrics Appointment at Jacksonville ASC on March 13, 10:00 a.m.)

03-08-2018: Biometrics complete

04-05-2018: Case status updated - Interview Scheduled on May 10, 2018, 10:15 a.m. :D

05-10-2018: Citizenship Interview - Passed English and Civics Tests, Recommended for Approval! :D 

06-19-2018: Received email and text notification: Naturalization Ceremony Scheduled; waited for letter to be uploaded on online account - it has been set on Wednesday, July 25, 3:00 p.m.

07-25-2018: I am now a U.S. Citizen!

Hi there! Welcome to VJ :) Please do not type in all-caps because it is hard for folks to read and sounds like you are shouting at us.

If you do a search on the forum, there is a lot of information about the vaccination supplement form, which is completed by a civil surgeon, although some people opt to just send in the DS-3025 which sometimes works and sometimes causes folks to get RFEs.

Naturalization

9/9: Mailed N-400 package off

9/11: Arrived at Dallas, TX

9/17: NOA

9/19: Check cashed

9/23: Received NOA

10/7: Text from USCIS on status update: Biometrics in the mail

10/9: Received Biometrics letter

10/29: Biometrics

10/31: In-line

2/16: Text from USCIS that Baltimore has scheduled an interview...finally!!

2/24: Interview letter received

3/24: Naturalization interview

I-693 to a civil surgeon in the US and get them to fill out the vaccination section is the vaccination suppliment, if you dont send it you may or may not get an RFE for this.

im worried because my husband does not have his shot record. Is the form I-693 and DS-3025 the same thing?

If he doesnt have DS-3025 to put in the aos package I would recomend printing the I-693 from and taking it to a civil surgion and having the vaccination portion done just incase, but thats just me. Up to you really!

it is the vaccination copy from the original k-1 medical that they would of given him. Hence if he did not get one he needs to get the I-693 filled out by the civil surgeon.
