We are filling out documents for my wife to change from student to spouse status.

Because she did not have a middle name when she was born in South America, she was not able to have a middle name on the license.

We pre-filled the documents with her her maiden name as the middle name.

Do we need to redo all of the forms to match the marriage license?

We will file a name-change notification with the county court in the near future to create a middle name.

Middle name is not the same as maiden name.

Many people have 2 names and 2 surnames. If she doesn't have a middle name you shouldn't write anything, and you definitely shouldn't write her last name there, cause that's not her last name and it doesn't match whichever documents you have

Well, we have a new SSN being issued which has her middle name as being her former maiden name. It will be fine I would suspect being that the federal government tax identification will be with her maiden name (her dads lastname, not both mom and dads) as her middle name.

Is that incorrect to think?

Then what's her last name? I'm really confused. :P I'm from Uruguay and I have 2 names and 2 surnames (My dad's and my mom's). So I wrote my First name as first name, my middle name as middle name and both surnames as last name, cause that is what appears in my passport and my birth certificate.

If you're using any of this to apply I would change the forms to match that of the birth certificate/passport. But I don't know if what you're doing is incorrect, it might be just right.
