Hello this is my first post but i have been reading this forum during all my inmigrant application for my wife. Well my question is very simple but it confuses me. since the beginning when i first filled the petition for my wife, I-130 I used my wife's maiden name as her last name(since in Peru married women do not change their last name, they only add their husbands last name at the end of their full name). Now that im filling the DS-230, question 1 is asking me for family name and number 2 for maiden name, i know her family name will be her last name, but her maiden name will be the same. So i called the NVC and ask about this, they just answered "put whatever name is in her passport" which confuses me a lot more. so here is the example: her name as single was Maria Reyes Campos, and now her name as married woman(on all peruvian documents and passport) is Maria Reyes Campos de Perez. As you can see, her maiden name is still her last name. So im thinking of putting my last name Perez as her family name and to put Reyes as her maiden name, but im afraid since all her peruvian documents including passport have her maiden name as last name. So please if somebody can help me i will be really happy since I already have all the documents ready to send. I only need to answer this question on form 230 thanks in advance

I would do as they say and put the passport name to avoid confusion on their part. When she gets to the states if you want her last name to be your last name you will have to go down to your local town/city hall and fill out a name change form. According to the US, her official documents will have her as Maria Reyes Campos de Perez, "Campos de Perez" will be listed as one last name. I know, it's wacky here. You can have it changed to just "Perez" when she gets here, it will be much easier to get it done here than down in Peru :wacko:

And don't worry her passport from Peru will still be valid. My husband's documents in the states have one last name and the passport from Peru has two and he has had no problem traveling back and forth.

so the means in question 1 in family name i would put Reyes Campos de Perez and in question 2 in maiden name i will just put Reyes Campos right?

You're getting confused with the Campos (her mother's last name) which won't be used in the US. As far as the US is concerned she will be known as María (middle name if she has one) Reyes. The "de Pérez" is not used here unless you want it to be and put it down as Reyes de Pérez, as if it were only one last name.

So, her family name is María Reyes. Her maiden name is María Reyes. If she wants to use your last name in the US you will have to fill out form DS-230 now as María Pérez. If you wait to do it in the US, you will have to file form I-90 to have her green card replaced and pay another $370.
