Hello everyone! I arrived in US last March 23, 2010 and we got married last April 8. Right now I am working on my AOS documents.. My questions is this.. Do i need to fill up form I-163? according to the I-485 instruction, if a k1 fiancee had a medical examination within the year i only need to submit the vaccination supplement together with my I-485. But in some sources it says i need to fill up the first portion of I-163 and need a civil surgeon to sign the document.. Please give me some ideas.. Thanks VJ'ers!

May 11, 2010 - NOA for I-485,I-765,I-131 May 17, 2010 - Biometrics appointment received in the mail

May 24, 2010 - Walk-in Biometrics successful (ASC Michigan City, IN)

June 1, 2010 - Case transferred to CSC

July 14, 2010 - EAD approved, for card production

July 19, 2010 - Received AP in the mail

July 21, 2010 - I-485 GC approved, for card production

July 26, 2010 - Welcome letter received

July 28, 2010 - EAD card received

July 31, 2010 - Greencard received

Removal of Conditions

May 7, 2012 - Date Filed I-751

May 9, 2012 - Date of NOA!

June 8, 2012 - Biometrics Schedule

If the medical is less than 1 yr old than the I-693 only needs to be filled out at the top and then taken to a surgeon to transfer the vaccines from the DS-2035 to the I-693. Getting a surgeon to do this without some hassles is always fun. There are plenty of threads in VJ about this. I suggest that you have the instructions printed off and show them to the surgeon that only the transfer is needed.

Thanks for the advice! Yeah i show them the instruction and finally after long discussion.. the clinic figure out what they supposed to do.. got the I-693 done yesterday. :dance: Was a bit pain in the butt looking for a civil surgeon who doesn't charge too much fee. Glad another form was done.
