Preparing my AOS documents

Hello everyone! I arrived in US last March 23, 2010 and we got married last April 8. Right now I am working on my AOS documents.. My questions is this.. Do i need to fill up form I-163? according to the I-485 instruction, if a k1 fiancee had a medical examination within the year i only need to submit the vaccination supplement together with my I-485. But in some sources it says i need to fill up the first portion of I-163 and need a civil surgeon to sign the document.. Please give me some ideas.. Thanks VJ'ers!

If the medical is less than 1 yr old than the I-693 only needs to be filled out at the top and then taken to a surgeon to transfer the vaccines from the DS-2035 to the I-693. Getting a surgeon to do this without some hassles is always fun. There are plenty of threads in VJ about this. I suggest that you have the instructions printed off and show them to the surgeon that only the transfer is needed.

Thanks for the advice! Yeah i show them the instruction and finally after long discussion.. the clinic figure out what they supposed to do.. got the I-693 done yesterday. Was a bit pain in the butt looking for a civil surgeon who doesn't charge too much fee. Glad another form was done.
