Hello everyone..

DS-157 #12 question, list your last two employers.. my problem is i don't remember my supervisors name and telephone number, it was a 3 months project base job some 3 years ago. any advice will be greatly appreciated.. thanks in advance.

17-Feb-10:Send 129f to CSC

19-Feb-10:NOA1

22-Feb-10:Touched

05-Apr-10:Touched

05-Apr-10:E-mail received case approved(waiting for hard copy)

06-Apr-10:Touched

09-Apr-10:NOA2 Hard copy received

20-Apr-10:NVC letter received (Dated April 16)

24-Apr-10:USEM letter received (Dated April 15)Funny!USEM Letter Dated April 15, and NVC Letter April 16.

16-Jun-10:Medical....PASSED

24-Jun-10:Interview (7:00AM).. PAAAASSSSEEEEDDDDDD...

I would put it on there. All these documents are to be filled to the best of your knowledge. Try and find out his name (maybe old co-workers would know? If you remember the project name, etc. there will be documentation on who carried out this project somewhere.. I know it's a pain). If you put 'Not employed', they could find out and ask a lot of questions...

Also, for everyone else, isn't the DS-157 only for male applicants between 15 and 55 or something like that?? At least that's how it is here. I couldn't find anything on the webs... does anyone know specifically?

Thanks for the advice, but i found out that the supervisor that i worked with is no longer connected to the company, what will be my options?
