1. My fiancee already filled the I 134 that she printed out from the USCIS website and had it notarised. However I read on another thread that I´m not allowed to turn that one in but am supposed to have her fill out the one the embassy sent me. The form she filled out is almost identical to the one they sent me, the fields are the same, but layout is different and #11 has a missing "do" in the "That I do intend". Also the form she filled out seems to be more recent than the one they sent me, her´s being OMB No. 1615-0014; Exp. 04-30-07 and the one the embassy sent me being OMB No. 1115-0062 and not having an expiration date.

2. This is paragraph on the checklist is confusing me "FOURTH: As soon as you have obtained all of the documents that apply in your case, carefully read the statement at the bottom of this page, sign and date it, and return the checklist to this office. after the form has been returned to this office, you will be scheduled for a visa interview at the erliest possible date. You should bring all of the above pertnent doocuments and the application froms with you to the interview".

So A. Do I have to wait to do the Medical Examination before turning in the checklist? It´s scheduled on the 15th this month which was the first i could get an appointment. The waiting time I can expect from the time I turn in the checklist until the interview is 21 days.

And B. Judging from the quoted paragraph it looks like it to me that I´m not supposed to turn in any of the forms with the checklist, what do you guys think?

I think that there's no problem in filling out the I134 form that's up on the USCIS site. Some of the forms the consulate send us in our packets are outdated. You should be fine. If still in doubt, contact the consulate and ask.

It seems to be that you have to inform the consulate that you have everything asked for in the packet. If that includes medical thing, then return the checklist *after* having it done.

I don't know what it's like for you in your country, but in Brazil we're not supposed to mail anything to the consulate (medical exams, police certificates, etc.), but a copy of the beneficiary's birth certificate, a passport picture, DS230 part I and DS 2001 (that informs them that we have gathered all documents).

I only offer advice - not even legal. Just the plain and simple kind.

Sorry, I can't help you with Question 1 since we only used the forms which were sent to us. With regard to the rest of your questions.... You do not need to wait to do the medical before returning the checklist. And, no, you do not return any of the forms with the checklist. They will ask for those at the interview. Good luck!
