DS-2053 AND I-683

i don't really have DS-2053. its just probably not given to me when i had my medical in the philippines. but since my medical was already more than one year, i went to a civil surgeon did the medical and results are in I-683. he didnt put my A# there though. i wrote it in first page but the CS didn't write it in the specific page where he is asked to fill out. so, i only submitted I-683 with my AOS packet. i wonder if these is alright?

You will find out if they RFE you, but its likely fine the way it is.

do you mean the DS-3025? ...you're a K1 so you don't have an A#, and the medical form is the I-693 and the DS. anyway, i hope the cs put the results of your medical in a sealed envelope, as required and you don't get an RFE

er, K1's do have A#'s...anyone who immigrates gets an A#.

i have an A#. yeah, it's probably the DA-3025. I don't know where I got the DS 2053. Yes, the CS put the forms in a sealed envelope. I have the photocopy of it too.
