This form seems very straight forward but one thing bothers me. After you put your address and the Service Center address, in bold red letters it says" or when submitting for interview". Below this they ask for the name of the Country where Embassy is located, the address of the Embassy and then a (date mm-dd-yyy). Can anyone explain this form to me?. I am a little puzzled. Thank You.

The example letter of intent is written to show you the two 'versions' you will need. The first is for when you submit one (from each of you) to USCIS along with the petition. The second when you both send another for the interview stage.

Both examples are shown on one single page. For the USCIS stage - delete the references to Embassy - for the Embassy stage, delete the stuff for USCIS. Make sense?

if you're talking about the example form here in VJ, is the date when you submit your application (I-129F) or when you and your fiancee sign the letter.

Hi Yakad, right now the form will go with the I-129f package, so the information is needed for the address of the service center in Vermont. When they approve the petition(and they will!!:)) then the form needs to be filled out again, this time with the address to the Embassy. Thank you for all of your help, I still may have some questions in the future to ask:).
