Packet 3 Preparation

Quick question, would anyone know what forms I should begin to gather while we're waiting for the NOA's to bounce around? From what I've read so far (here and there) I'll need to have my police check (what type specifically?) and an extended birth certificate (not the typical wallet sized one). Any others I should be prepared to gather?

Thanks in advance!!

Don't forget a previous marriage and divorce certificate if you were married already.

As for a police check, you need a regular national/CPIC check available at any police station where you are. The RCMP and OPP can provide those to you. They generally take anywhere from a few hours to a few weeks to be completed.

https://www.orgforms.gov.on.ca/eForms/start.do?lang=en <---Where you can order birth, death, and marriage certificates. I just ordered mine and received it last week. Make sure you select "long form" when getting the birth certificate.

Unless you have ever been arrested or have any criminal record, then you will need the RCMP fingerprint version of the police check.

Do you have a record of all of your previous immunizations?

I have never been married (he has and his divorce decree was sent with our I-129F packet), nor do either of us have a criminal record.

I do not have an updated immunizations record, I thought I can get that during my medical DS-3025 form?? If not, might you know of a link that provides a list and I will run out and get those taken care of.

Thanks for the information. I'll stop by Peel Regional Police and have them print my CPIC thingy and I'll print out the long form birth certificate tonight and mail that off.

Yes, here is a list by age group:

http://www.panelphysicians.com/Pages/Immunizations.htm

If you have had the vaccinations but just have no record, you can get a titer test done to prove immunity, instead of having all the vaccinations redone.

Better to do all this in advance, the panel physician will just give you all the immunizations and make you pay for them.

Ok, so I'm a wee bit confused about the immunizations.

From what I read I need another Tetanus/Booster shot 4 wks prior to my medical? So the one we all had as kids, or after an accident to subdue any possible infections doesn't stand?

I get the one for the mumps or measles. Most of us had those shots as kids.

Chickenpox? Do we really need a shot if we've already had the chickenpox??

Hep B, Influenza and Mengingococcal didn't specify that a 32yr old would need them. So I guess I'm only really concerned about the Tetanus, Measles and Chickenpox.

And Spoom, aye, pretty sure Peel will do that for me. I have friends that have required it for work related purposes and they've gotten the same thing. $40 in and out, same day.

You NEED your TB shot every 10 years. They only last about that long. I'm not saying this for immigration, I'm saying this for your health.

You can just tell the physician you had the chickenpox, they accept verbal confirmation.

Ah, ok. Point taken.

Well, I made an appt to see my family doctor this Thursday. It's not to take the immunizations per se, but to sit down with him and discuss what is happening and what he can help me with as far as preparing for the medical. Surely he might have some insight.

There really isn't much to prepare for unless you need a teeter test.

You kinda just show up really early, give them some money, take and xray, give some blood, get examined, present your immunizations and are asked LOTS of questions.

Would the amount of the medical exam be dictated in packet 3? Or is there a website that indicates the cost? I tried looking at the link provided above for a cost, but I was unable to see one.

It depends on where you go. I see you're from Toronto so you'll probably go to Dr Sneiden.

It's 275 for the exam and then 50 extra if you want the results delivered to your house.

Actually all that information is on the website I linked to above, including costs, when the packet is available to be picked up etc.

Appointment Times:

All appointments occur between 7:00 am to 8:30 am on Mondays, Tuesdays and Wednesdays. You must make an appointment for a particular day. You may come any time between 7:00 and 8:30 am.

Documents:

If you do not wish to return to pick up your documents, which will normally be available in 48 hours, you may have them sent to you by courier. The cost of the courier is $50.00 anywhere in Ontario and $125.00 outside of Ontario and anywhere in the United States. You may elect to come back and pick the documents up on the day they are ready (two days after the medical) or any day after that is more convenient for you. You may also have another person pick them up for you, if you indicate so on the day of your medical.

Scheduling:

If there is some difficulty in scheduling an appointment during the above times, call our receptionist at 416-362-5603 or (toll-free) 1-866-362-5603 and we will endeavor to make another arrangement for you.

I would hold off on getting your Police Cert. until closer to actually getting the Packet 3 since it is only valid for 1 year and you can walk-in and get it in Peel.

In Peel there is only 1 station that does it - It's in Brampton, I forget exactly where, but I think it's Hurontario and something... It took me about 30 minutes total on a Saturday and was around $45.

Also for your vaccinations, you must prove you had them so either have your Dr. write you a letter or you can also get your Immunization Record from Peel Region Public Health if you went to school in Peel. I just emailed them and they mailed it to me in a 4-5 days.

Removing Conditions

Sent package to VSC - 8/12/11

NOA1 - 8/16/11

Biometrics - 9/14/11
