Timeline

The civil surgeon I called stated my now wife's (k1) name had to match her passport. We are filling out the forms for AOS with her newly married name. Is the dr correct? Is it ok to have this filled out with her maiden name and all the other paperwork with the married name?

That was the same suggestion that the CS's secretary told me. I presented my marriage certificate and it shows my new name so she didn't insist.

Make the names in all of the forms the same.

I'll give it a try, but over the phone they didn't seem to think its possible. Hate to look for a new CS because i called about 20 of them yesterday getting rates and what not. Every person has different rates, different rules, half of them are rude..it really is terrible.

Actually i called the court house and can pick up my certified copy first thing in the morning, so now its up to the CS to help me out or not i guess.

They should do it, if they give you any hassle ask them why the SSN office will let you have the name changed with the passport in your maiden name and your marriage certificate for proof.
