Sorry if I am re-hashing an old topic but I couldn't seem to find any information on what relates to my situation.

At the time of taking my medical in London I did not have the required Tdap and MMR vaccinations but was told I could get them done once I had the visa and was stateside. Rest of my K-1 process went smoothly and have been here in the states, married just over three months now I am now applying for my AOS. I went and got the vaccinations at my Central Health Center and sent in the stamped and dated immunization history along with a copy of my Vaccination Documentation Worksheet. I now realize that I should have filled in the I-693 although this is where I am getting confused. I am not 100% sure what sections of the I-693 I should be completing or if that is something I am able to do in the first place seeing as I had the two shots since I have been in the states.

I ave called USCIS and the operators just keep telling me that they do not ave access to individual cases. As I said earlier, I have taken a look through some topics here but can't seem to find anyone in a similar situation as me.

If anyone can set me straight it would be much appreciated otherwise it will mean getting another medical just to be on the safe side!

Hi Adam,

You don't have to worry about which parts of I693 you have to fill out, the civil surgeon will do that. All you have to do is find a doctor who is a civil surgeon, make sure you tell them you only need your vaccination documentation sheet transcribed onto I693, ask them how much it will cost. Some doctors will try to rip you off, make you believe that you need another medical. But tell them you dont need another medical, just I693 with vaccinations transcribed. They should be able to do it for cheap. Then just bring your proof of vaccinations you received to your appt, and they'll do the rest. You'll receive a sealed envelope from them, and you send that to USCIS. Hope that helps. Thank you for the advice, really appreciate it. The CS in my area wanted $125 to transcribe it. Does that sound about right?

No, that is expensive. Twice what I paid, and I felt ripped off!

You fill out Section 1 (about you) Don't sign it - the CS might want to see you do that. The CS fills out Section 2, part 5 (?) - the vaccination portion only. It's a table which looks a lot like the DS3025 from your medical. They also do section 5, which is the civil surgeon's certification of the transcription.

If you live in one of the 10 or so states where people have reported a CS who transcribes for a reasonable fee, you can find them in this thread. Otherwise, you'll just have to call around. Be firm, and if it sounds like they don't know what's going on, make them an offer (less than $50.) ;)

Yeah, I paid 35 dollars, so that's definitely way too much, you can probably get someone else to do it cheaper. When I was doing mine, the nurse at the doctor's office did everything, I just signed one piece of paper, but of course, everyone does it differently

That's awesome, thank you! Just called a doctor on that thread and will be heading there Wednesday. $40 for the whole thing, so much better then the original $125 I wa quoted. Thank you both for your timely and informative responses!
