Hi everyone! I need help. I have been trying to read some related threads about civil surgeons and filling up the form I-693. My medical exam which was done before I left my home country is still valid because I had it 2 months ago (as per Federal Regulations). However, I downloaded a list of civil surgeons in the nearby South Florida area and I tried calling almost all of them but it seems they really require me to do another full medical exam and it costs too much! My husband and I explained over the phone about this specific Federal Regulation that my medical exam is still valid and that they only need to transcribe my vaccination record (which I have from my med exam in Philippines) and they just have to sign it! But they said they cannot certify or sign the Part 5 since it would be a false certification considering I will not be examined and evaluated by them.

So at this, all my paperworks for AOS are ready, but I just need to accomplish this Form I-693. Do you know of a civil surgeon here in South Florida who can transcribe my vac record to my I-693 without requiring a full medical exam? I really need to know. Please send me address and contact number so I can mail my packet ASAP.

05/22/10 EAD in mail... FINALLY!

08/12/10 AOS touch...

09/09/10 AOS touch...

09/10/10 called USCIS and got a service request for AOS application...

09/13/10 email and text notification... AOS APPROVED and card production notice at the same time!!!

09/20/10 received green card and welcome letter in mail.... at last!!!

Civil surgeons have a license to steal. Try looking for the USCIS regulation stating the physical from your home country is good for 1 year. I know it is in the regs somewhere but my old link is now broken.

are you sure you got all your vaccinations completed in the Phils? usually there is or two that needs to be completed, and even one or two that was not administered to you there. the job of the CS is not just to transcribe but to check as well if you really completed the required vaccinations and assure the govt. of that fact.

better double-check your ds3025 and if it's indeed complete, bring that to the CS and show that no further vaccinations/medical exam is required for you.

also, print out the part of the uscis guide that clearly says "for K1 visa holders whose medical exams are still under 1 yr old no medical exam is necessary" so they won't have a reason to insist

When I had my medical in the Philippines, the physician asked me if I wanted to get the vaccination shot that I needed before I leave so I wont have to worry about anything when I get here. So I had it and it was completed in my record.

Thank you for the replies. And I will try to look up for the local health department in the area and check if they can transcribe it. I just have to make sure the one signing it is listed as civil surgeons of USCIS.

hey!!! im in the same boat as you, and have had the same argument with 5 different medical centers! its not just you don't worry :) but i have managed to find a place in Naples, FL which will transcribe the vaccine records. its the only place in SWFL i could find. Got my appointment booked for tuesday, so i'll be able to tell you the exact price then, but the receptionist said it would be around $20... super reasonable!!!

PS: i know its an extra journey if you're far away but i found being there in person to request an appointment helped me. the receptionist still tried to convince me to get a full medical re-done!

A big thanks Jo! I will try to call the clinic. Although its a little bit far from where I live, it will be worth the drive because a lot of civil surgeons in the area nearby are charging me hundreds of bucks and they really require full medical exam again! Wow! A very good business for them! Thanks to USCIS!

Hi ! I would highly recommend you Dr. William S. Hitch but he is too far from your place..His office is in Savannah. I just had my I-693 completed by him (on Feb 3, 2010) and he did not charge me anything. I was really amazed and thankful that I saved my money. I had my medical on March, 2009 and vaccination at local health department. I then take these records to Civil surgeon and explained that I dont need to undergo medical again as it is still valid. They processed my I-693 and Dr. Hitch signed it...for free..
