I-94 question

So my fiancee arrived and looking through her passport I noticed a few things. Please help me understand a few things:

1. they only stapled the little "Departure Record" piece to her passport.

2. At the bottom (same departure record form) of this piece there's questions asking for: PORT, DATE, CARRIER, FLIGHT #/ SHIP NAME

Is this a normal process??

The IO only stapled the lower half of the I-94 to my passport and he kept the rest. This part has my admission number, family name, first name, and country of residence.

The portion with the questions on port, date, flight number, etc. are not with me. That's the upper half the IO detached from the whole I-94 and it's for the government records.
