These forms all get so confusing. What do I have to return to the US embassy in London... ALL the DS forms and the checklist or just the checklist?

Does 'duplicate' just mean I have to send 2 copies of all the DS forms? It doesn't say on the website that DS-156k has to be in duplicate, it just says DS-156 and 157..

It doesn't say anything about sticking the photo on the form before sending which I read some people on here talking about...

Also, not all the forms can be filled out online, is this correct, 157 had to be printed and written on... is this how it is or did I do something wrong?

So, I only send these forms and nothing else? What about the police certificate? i don't have that many other forms, only my birth certificate and the evidence stuff.

Usually you send in the forms that the checklist tells you to fill out and send back.

Duplicate usually means to make copies (2x)

If the form requires pictures (the G-325 for example), it is a good practice to write the persons name on the back, and place them in a plastic baggie, attaching that via paperclip to the form.

I am not sure what you mean about the 157 - do you have a link?

Depending on the embassy, they request different form, usually they want to police certificate sent with the forms mentioned above.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

The US embassy instructions on the website says do not send in any documents, so I assume it means the documents I have to gather together before the interview.

It doesn't explain very well whether to send only the checklist or the checklist and all the DS forms..

This is the 157 form, it says send the DS156 and DS156k in duplicate but doesn't say if they only need one or duplicate DS157

Try this one for the 157

Ok - I look over the Uk site, you keep the support documents with you (for the interview) and you send all the forms you filled out to them prior to the interview (via mail) (this includes the document checklist)

It does say you need a duplicate 156 and 157.
