Hi guys. I'm going to be filling out all my AOS paperwork this weekend and am getting confused with the I-693. My medical was done in Montreal in august so its definitely still good so I wouldn't need the physical or blood work redone. So do I just need the civil surgeon here in Florida to do the vaccination supplemental (i-693a)?

If I do need the actual I-693 done by the civil surgeon, what parts do I need?

I just sent the sheet that Montreal gave me without having it transcribed and wrote them a nice letter attached saying I already had a valid medical. They accepted that and approved me with no RFE. It seems to vary by state/person though...

Yes if you have had your medical done in Canada then you do not need to redo your entire medical. If you are willing to risk an RFE which also means waiting longer fo interim benefits then you can just send in a copy of the vaccination sheet that Montreal gave you. I had my civil surgeon transcribe the vaccination sheet from Vancouver onto the I-693. Cost me $50, but worth it for me. The parts that you will need, the rest you can throw away will be Part 1, Part 2 & Part 5.

oh gosh. i just got my I693 done by mid december. i guess she retired for 2010.

I have called 8 civil surgeons in my area and ALL of them refuse to give me a partial except one who wants to charge me 200$! You've got to be kidding me!!!!!!!! There is no way I am paying that much.

Minnew, you said Part 1, 2 and 5. Part 1 and 5 I understand but Part 2 is like 4 pages long....would it just be the vaccination page sheet of Part 2?

I got the same run-around from several civil surgeons before I found one who would do the transcription. I did however have to be very clear in what I wanted. Your medical that you got is good for 1 year, so a civil surgeon requiring it as a pretext to getting the transcription is just them trying to squeeze more money out of you. I was told, by both our interviewer, and by the civil surgeon that they require the transcription to be done in person, meaning that Mel had to go to the civil surgeon to submit and sign the form in their presence to ensure that it was not being done fraudulently. I don't see how getting it done by an out of state doctor by mail/fax wouldn't be rejected given those restrictions.

Many people, probably hundreds, had theirs done by Dr. Arnold before she retired through the mail, so it's definitely possible and has been accepted. So, never say never!

If you don't have anyone in the area, I would just risk it and send in the worksheet you have. That's what I did out of laziness and it worked out fine. The worst that can happen is you get an RFE and are delayed a couple weeks.

After reading the instructions that come with the I-693, I can see how the doctors would think that they are required to give the medical. The instructions read as such:

After completion of the medical examination, the civil surgeon is required to give you, the applicant, the completed Form I-693 in a sealed envelope. Do not accept it if it is not in a sealed envelope. USCIS will return the form to you if it is not in an envelope or if the envelope has been opened or altered.

You, the applicant, not the civil surgeon, are required to submit the Form I-693 to USCIS.

If you are applying for adjustment of status: Submit Form I-693 together with your Form I-485, Application to Register for Permanent Residence or Adjust Status, as detailed in the instructions to Form I-485.

It also says that you are required to attend. I would wager that many of the interviewers or personnel who review the files don't even look at the address of the applicant versus the address of the civil surgeon.

$200!!! That's ridiculous!!! That's what inky was quoted when she called! I called the same place a few weeks later when doing my AOS & they said $50 or less!!

Yes it would just be the vaccination portion of Part 2 if the CS is transcribing.

You can risk sending it without RFE some people do not have any problems, but it's hard to measure the risk. If you end up being transferred to CSC usually no problem approves without

interview. If you end up having an interview & did not send the 693 transcription you still have the same dilemma as now, trying to find a CS to do a transcription.

Actually is says right in the instructions for the I-693 regarding K-1 and only needing vacc. suppliment done. We took a copy to the civil surgeon and once he read it, he had no problem just doing that portion.

i was quoted $300 from where I live. there is only 1 civil surgeon and everyone else is 1.5 hours away. I explained to the civil surgeon that i don't need a full medical exam & just transcribing I693 but they said for me to be a "client" they need to do a full medical exam and have to pay the copay and administration fee b/c i was not a current client.

I had to call someone else in Tacoma, WA who has done many I693 and they quoted me $50.

The quote I included in my prior post was a direct quote from the USCIS site's instruction page for the I-693. It is worded quite poorly and could be taken several ways.

You need the I-693 vacc supplement unless your DS-3025 is complete. Part 1, part 2-the vacc supplement, and part 5 of the I-693. They should have given you that at your medical. Just look down in the left corner and see if it says COMPLETE. Mine actually said incomplete eligable for waiver based on age appropriate. They would not accept mine as it was ticked off not complete. I think that's where the RFE's are coming in on people who are just sending the DS-3025. If its complete I don't think there is an issue. But that's just my opinion. We are waiting now on the whole TB skin test as my medical was done in Montreal and they did the chest x-ray not the TB skin test. So we are thinking we may get RFE'd for that. The website says you must have a TB skin test, but I know people are being approved with just the chest-x-ray. The TB skin test is not expensive, $10 bucks here at the local hospital. We should have had it done before we sent the RFE back it. Its a waiting game now....
