Hi everyone,

I'm trying to find more info on I-693 and I've been confused by a lot of Civil Surgeons' receptionists in NC. I need your help, VJers!

I have obviously done my medicals back in my country and they're filed within a year ago. I do have my Vaccination Documentation Worksheet, which I believe is the DS-3025.

According to most of the Civil Surgeons I called, they tell me for the new I-693 (revision 10/14/09) that I have to go through HIV tests, Syphilis and also the TB Tests... is that correct?? These adds up to about $150!!!!!! Do I really need to do these tests?

Also I have questions regarding my vaccinations: I do not have "Td" vaccination as it's not routinely available. I guess I would need that??.... also, for Varicella, mine is written as VH (Varicella History), does that mean I don't need it anymore?

According to I-693 instructions below:

Section III. Frequently Asked Questions

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e), or as a

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen, and

C. You received a medical examination prior to admission, then-

You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination.

You will, however, be required to complete Part 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated civil surgeon must complete the vaccination section and Part 5, the Civil Surgeon's Certification.

With the above statement, does that mean I really do not need all the Syphilis, HIV and TB exam? But, only to complete the vaccination section?

Hi!!! I did the same thing yesterday. At first I was having second thoughts of not having the blood test and TB skin test since it was stated in the instruction that we don't need to do it as K1 had medical already but then I just decided to do these since some were getting RFEs of not doing the full medical exam wid the CS. Regarding the tetanus, our CS did not require me to get tetanus since i got the Tdap already. Anyway, he will be the one to do the paperwork. We paid 135.

I had the same argument yesterday with the receptionist at the CS. I went ahead and paid the 175 they charged cause we need to get the ball rolling. They made my wife do HIV and TB now we have to wait til Monday so they can read the TB test. I think USCIS should inform the CS that we do not have to go through this again, it's ridiculous.

I called this morning, the TB skin test here is only $10, after Jan 4th they are longer doing HIV testing, and the others I am not sure of. We sent back our vacc. portion of the I-693 and are waiting to see if we get another RFE. I only had a chest x-ray for the TB so we are expecting a request for a full medical.

When i did my medical in the phils i was told i only need to get one more shot which is the td shot... but when i went to the civil surgeon's clinic they told me i need another shot of the mmr...

we paid 125 total.. transcription $40 and for 1 mmr shot.... i got the td shot for free at the base

I just mailed the I-693 from the clinic, i had the physical and bld test and tb test. They told me they cant fill up the form if i dont have to do everything they need, so to get done we done everything and cost me 115 plus the 40$ xray. Oh coz they said im positive with the tb test i have to have the xray again and then again its negative. Thanks god. I hope im going to get my working permit soon...

hmmm... sometimes i wonder why there's an instruction when they're not exactly being followed by the USCIS!??? i.e. I-693 instructions clearly stated only Vaccination portion of Part 2 is needed if I-485 is filed within a year without any Class A or B medication issues! Sigh.... frustrating :/

Oh! And I have another question.... is it possible to do the I-693 before marriage???

If your going to have a second one, or just the vacc. portion then yes you can.

I believe all you need is the Tdap shot but I have been hearing that some CS asked for flu shot too since it is the season now. You don't need to do the HIV/Syphillis since you still have your K-1 Medical which is less than a year. You don't need Varicella. Some CS will try to force you to do the TB Skin Test and since we had BCG vaccine in our system, the test will turn out positive and it is a requirement in the state of NC to report to your county's Health Department.

As for doing the I-693 before marriage.. I guess you can. Make sure you put the same names on the name fields (especially if you have changed your name) on your I-693 and the rest of the AOS forms.

I did my full medical here in Charlotte. Paid $230 for HIV/Syphillis, Tdap shot, TB Skin Test and physical.
