I have read thru the guides and all of the threads, but can't find the answered.

1) can you give me the breakdown of the total AOS fee of $1010

2) what is the difference in the I-693 and I-693A?

3) The lockbox in Chicago...is this a PO box type? The reason i am asking is that you can't send fedex or UPS to a PO box number? These 2 companies have better tracking system than USPS.

4) My CS said that my wife will need the chicken pox shot...is this correct? I did not see this requirement in the new 2009 requirements.

Form I693 is the form used to get the “whole medical exam” while the form I693a (or known as the Vaccination Supplement form) is for medical result transfer (a certain AOS applicant took from his/her country) purposes only.

Check the instructions - there is an address to use for courier services.

Q: What are the required vaccinations?

Vaccinations are required as part of the medical examination. The law requires all individuals applying for status as a lawful permanent resident (either by applying for an immigrant visa abroad or for adjustment of status in the United States) to establish that they have been vaccinated.

The required vaccinations are:

Mumps

Measles

Rubella

Polio

Tetanus

Diphtheria toxoids

Pertussis

Influenza type B

Hepatitis B

Any other vaccinations recommended by the Advisory Committee for Immunization Practices (ACIP) (Chicken Pox falls under this - My CS required Umit to get it also.)

Current ACIP recommendations also include varicella, haemophilus influenza type B, and pneumococcal vaccines

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

It's $930 for the I-485 (will also cover your I-131 and I-765 if you file them together with your I-485) and $80 for the biometrics fee. I'm not sure sure what's the difference with I-693 and I-693A but I think the I-693A is a supplement to the I-693. If your wife has no history of having chickenpox then she has to get the vaccine as it is listed on the required vaccines on the I-693. We used UPS to send our documents but not to the P.O. box address.

Let's see what the other more experienced members here has to say..

thanks...

The USCIS fee schedule did not mentioned exception for I131 and I765...that was the reason why i was confused regarding the $1010 total.

I downloaded the I693 from USCIS site...but did not see the I693A....is it part of the 693 form? My wife does not need the full physical

It's not part of the I-693.

you won't have to - look closely at the I-693, it has seperate sections for the medical and vaccinations - your CS will fill out what you need.

In Umit's case, they just filled out the vaccination section and we sent that in.
