Good days to all of u here...i need help and suggestions regarding of my problem of my birth certificate

my parents were not married and the time i am born i used my mother's surname but when i entered schooling i already used my father's surname, my credentials in school and all the ID's i have are all with my fathers family name...............because my father file for Certificate of Acknowledgement last year 2006 stating that me is his daughter..i am a illigitimate child stating my birth certificate..i went already the municipal registrar with my father last year 2006 my father file for certificate of Acknowledgement..it is state there of my new record the remarks "The surname of the child is hereby changed from "Tambis" to "Regodo".But when i got NSO again last year 2008 it doesn't change my lastname still state there of my mother surname...what should i do?please i need help....

And i found other problem of my live birth is the gender it is written male in my NSO..that gender written a male it need correction that i am a female..i can't get passport..

And how long it wil take for both problem of my surname and gender to process?and how much it cost?

I am sorry about your situation. you should go to the main office of the nso and ask about your situation. do it as soon as possible, because it will takes time. Goodluck!

Hello. My birth certificate was also corrected in Quezon City Hall in 2002 (with annotations) but when I requested in 2009, it wasn't changed because QC registry office did not forward my document to NSO. Maybe that's what happened to yours. You have to go to your local civil registry office and ask them to endorse your document to NSO. They will need your Certifcate of Finality and other docs regarding the correction of your BC. It took almost a week before they gave me an endorsement letter because they had to look for my record manually. I paid 120.00 (I think) and then 140.00 at NSO. With regards to your gender, maybe you will have to go through the same process when your surname was corrected.

i am the one who sent the copy of my birth certificate to endorsed in nso about with annotations..but when i got NSO last 2008 it nothing change about my surname

Hi,

Your biggest problem is the incorrect gender, you have to go to your Local Civil Registrar where you were born and ask what to do, it is not easy to get the incorrect gender corrected. My ate has the same error sa BC nia, kailangan ng lawyer tapos wait pa decision ng judge, hindi rin sia nakakuha ng passport. Ung fiscal na nilapitan namin asked for 10,000.00 pesos para sa processing.

Yung annotation na dinala mo sa NSO, you have to follow up every now and then kung nasa data base na nila. Me problema din ung BC ko dati, pero madalas ako mgfollow up if nasa data base na nila, if possible kunin mo ung name na pinasahan mo ng annotation para doon ka rin mg follow up sa kanya.

thanks ..yes incorrect gender is the biggest problem...the private attorney asked me 20,000.. and 500 pesos monthly hearing...ilang months natapos ang proseso sa ate mo?about my lastname i went to local civil registrar last week ..there are some papaers hindi ko na attach sa pag kuha sa NSO..pina xerox nila yun..tapos ako nlang mag pasa sa NSO tacloban...

If you forwarded ur birth certificate with annotations to NSO,you should always go to AREA B(NSO East Ave) everytime u make a request,because that's where you'll get your annotated BC.I also had my name changed and used the RA9048 and whenever i requested for my annotated bc,i always go to east ave. Area B to get it,i applied before to other nso offices but i just got my uncorrected bc.
