i am applying for AOS I 485..do i neeed to apply for i 693? i had a thorough med exam at INDIA, and i have 3025 vaccination record..Kindly guide me how to go about i 693 form..

You need to find an USCIS appointed civil surgeon to transcribe your vaccination from DS 3025 to I-693. You can start from here:

If your exam in India is less than a year old, you don't need to redo your exam again. Many civil surgeon will try to convince you that you need to redo it again. Don't fall for that. You may also need to complete whatever vaccination that you are missing.

Some people submitted their DS-3025 and got approved but if you want to make it sure, have it transcribe to form i-693.

Please READ these instrutions, beginning to end. The FAQ section states that if you have a K1 visa and Had a medical exam overseas , you probably DO NOT need to fill out another I-693 UNLESS your DS-3025 (vaccination record) was filleed out incorrectly (the one in the sealed envolope). This MIGHT apply to you. Read the instructions, and you will know.

They were amended 10/14/09, so they are relatively new at this time. Hope this helps somebody, it would've saved us time, money and heartache!!

Moderator: Could you please look into amending item 11 in the checklist for AOS if you have a K-1 or K-3? My wife came in on a K-1 but your checklist states she MUST have an I-693 filled out ahead of time and sent in with our I-485. According to the I-693 instructions amended 10/14/09, this is clearly NOT the case, at least as I read it. Bear in mind, I am not claiming there is another, contradictory instruction lurking out there somewhere, but if this saves somebody some $$, time and heartache, it would be well worth your time to take a look at this. Thank you!

I sent I-693 even my medical is less than a year after filing the AOS. In my surprised I received an RFE requesting for I-693 again. I thought the reason was Dr. Arnold made my first I-693 but another 2 AOS Nov Filer also asked for I-693 again.

Some just gave the DS-3025 and received an RFE, some just asked to bring the I-693 in their interview.
