What confuses you about USCIS making you print your name next to your signature?

ps. Normally middlename means your maiden name (after you get married) or your mothers maiden name if you are single. Second name is not the same as middlename. ERIKA FELICINDA is your given name. For some reason, the US is not used to people having a first and a second name. The second name always ends up being the MIDDLENAME or MIDDLE INITIAL on documents and/or ID's.

The 'Given' name is the first name. Family name would apply to the name you are currently using as your last name. However, you probably should not have used Felicinda and Pastor in different places for the middle name. As pointed out, it is common to use one's maiden name for a middle name. With that, consistency would be the better method.

Oh, one other thing. Congrats, hope you're happy, from an old friend .
