Hello all,

Just a quick question...I was fillling out my fiance's DS156 in the online form format given on the Embassy's web page. For anyone who is familiar, you type into a database type form online and after choosing accept, it produces a document for you with a barcode on the final page (assumably, meaning they file it). Unfortunately, I forgot to put the address of my fiance's school on the form. So, my questions are:

- Can I create a second DS-156? What happens with the old one if I do?

- Who do I contact to cancel the original?

- Will the school address matter?

- Can I just write it on the form?

- Any other thoughts?

Hope someone else has done the same and can help. Thanks!

You can fill out the DS-156 as many times as you want. Clicking "Continue" or "Accept" does not transmit any information to the Department of State, it just encodes the form data into the 2D barcode on page 3 of the form.

You do not need to contact anyone or cancel anything; once again, nothing was submitted to DOS so there's nothing to cancel.

If you left something off the form, fill it out again and print a new one. Do not write additional information into the form after printing, as that information will not be captured in the barcode.

Just out of curiosity, which question of the DS-156 requires a school address? Is the school her employer?

(DS-156 is used at the embassy/consulate stage - moving to Embassy and Consulate Discussion forum)

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

Relax... yes you can create another DS-156. They are not processing what you typed in - the electronic form simply codes your information into the bar-code that gets printed on the form (in other words, the data is not stored anywhere but on the form itself).

Great news! Thanks for the help everyone! As for your question, if he/she is a student, they ask for the school under the employer section.
