Who receives Visa fee in the packet? Agent or sponsor

Hello,

I have following questions regarding my case.

1. Whoever is appointed as agent, will that person receive the visa call fee? (fee that has to be paid when person gets medical exam packet, etc.) or is it always the petitioner who will receive the letter to pay fees?

2. I have received the Document Cover Sheet after paying $70 AOS fee. Do I need to wait for any other documents from NVC to send the I-864 or can I just print them and mail that with document cover sheet form?

3. Do choice of address and agent form has to reach NVC before I-864 or toghether or it doesn't matter?

I appreciate someones prompt response regarding the above questions.
