We used something like this for both the AoS process and the recent RoC process...my sister-in law who is a lawyer highly disapproved of the informality of our docs (though we asked friends and family to get them notarized), but I felt it showed some personal care/touch in what they wrote- and we kept the overall formalized structure. Just a suggestion of course from me -the form is pretty simple for friends to fill out. Good luck!!
