Hi everyone,

In the last 5 years I was unemployed 3 times So as not to leave any date gaps I need additional space as There is not enough space to place this information..... I know I need to write see attachment and the list everything there as I need 3 more lines than provided on the form. Do I have to make 4 copies of the attachment. One for each page or just one attachment for the whole 4 pages. And is it ok to staple it to the back of each copy if I need to have 4 copies of the list? Thanks

I needed more space for my husbands residence history.

I started writing the places in the lines on the form itself and in the last line I put"See supplement page attached". On the supplement page itself, I put his name and address as well as "Form G-325A Continuation of "Applicant's residence last five Years"". then I created a table similar to the one on the form and had my husband sign and date the extra page. I printed 4 copies of it, each signed and dated and put it behind each page of the G-325A. I just put it loose behind it, didn't staple or anything like that.

Most folks will simply make four copies of the supplemental page(s) - one for each copy of the G-325A. I (personally) would staple each to each copy of the G-325A.
