can anybody share me a sample of an affidavit.

im having a hard time with this.

You do not need to have them notarized, but I would recommend it if you want them to look more official. A notary is always an added step to showing USCIS that you are serious. Now this is a completely legal Sworn affidavit that has everything you need legally on it that the actual Removal of Conditions requests. Hope it helps.

To whom it may concern,

This sworn affidavit is to serve on behalf of (INSERT NAME HERE and (INSERT NAME HERE) as my written testimony to affirm personal knowledge of their marriage and relationship. i will NOT have it notarized though since it is not a must to have it notarized and as others did not have it notarized.

I have like 9 of them all notarized, but I am a freak of nature and I won't take any chances lol

I called the UCIS CS and asked if the affidavit needs to be authorized and the person I talked said "It don't need to". And here some sample forms from VJ EXAMPLE FORMS

It does not have to be notarized, but it looks more official and it validates it. It proves that the person signed the form in front of a witness. It's more solid in my opinion, but not required. If you have little evidence, then I recommend getting them all notarized despite the advice you get here today. Every little thing helps when you are low on evidence and I am not taking my chances... no way in hell

I have never been a fan of "notaries".... Yes i understand this is needed especially for business documents but for sworn statements on bonafide marriage?! Why should there be a need to pay somebody just to witness the signing of the affidavit. The friend making the affidavit is the "witness" to the relationship. The "sworn statement" is important, not the guy at the UPS store seeing u sign the form. Yes it looks more official but that doesnt mean the statements are true if its notarized. Just my 2 cents.

Honestly it's a small step, it's easy, it's free and it takes like 15 minutes of anyones time.... It's the least my family can do to help me so they were glad to get it done. If you lack evidence, then I recommend any small step you can take to help your case.

The wife's green card was just approved and received in the mail this week. We enclosed two letters written by friends who knew of our situation. They were each very different in form, and neither were notarized. Since these statements were at best "icing on the cake" documents, and not primary evidence of our marriage, I didn't feel the need to make the drafters of the letters go the extra step and have them notarized.
