My fiancee's son is 3 years old and I was wondering if she needs to take him to the interview? I know what the website says, but he is only 3. Also, what would they be asking him? Should I email the embassy to verify if he needs to come?

He is listed on the petition, so he would be coming with her.

From the Embassy in Moscow website:

Children immigrating with you must also be present at the interview.

Doesn't matter. My future stepson is 2.

Also, what would they be asking him?

"Hi nice boy, do you want a sweet?"

Or maybe, "What do you think about the current state of relations between Russia and the USA?"

Should I email the embassy to verify if he needs to come?

No, the instructions are clear.

He is listed on the petition, so he would be coming with her.

If the intent is for him to come over with her, they both go.

And don't forget he needs a separate medical examination and she has to pay for his application along with hers. Double the cost.

And don't forget to bring the original birth cert, a copy of it, and a translation of it. Plus, his passport if he is not listed in his mothers.

And the signed document from the father allowing him to go, if that applies.
