Hi everyone, I am getting ready to prepare the paper work for my fiancee. I was following the steps in VJ guideline, my question is re: the I693 the medical exam part. I was looking at the form, I didnt see the I693A. My fiancee had medical and vaccines prior coming to US as part of the requirement of K1

It stated just to submit the vaccine record if he already had the physical exam, now do we just make a copy of his vaccine record? or does he still need to visit one of the civil surgeon to have them fill out the form? how much does this usually cost?

Please any advices will be helpful to us...

thank you

I-693a is no longer used. Best to have your fiancee go to a civil surgeon and do the i-693. You can use Dr. Arnold in Texas. Does the trip by mail and fax for $30.

LOTS of peeps here have used him.

If it's been less than 1 year since he had the immigration medical he can use Dr. Arnold. If more than that he will have to have a full medical.

From the USCIS website:

Medical Examination

If you received a medical examination prior to admission as a K nonimmigrant, then you are not required to have another medical examination at time of adjustment as long as:

Your Form I-485 is filed within 1 year of your overseas medical examination

The medical examination did not reveal a Class A medical condition

If you did have a Class A medical condition, you received a waiver of inadmissibility and you have complied with the terms and conditions of the waiver

Even if a new medical examination is not required, you still must show proof that you have complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart, and Part 5, the Civil Surgeon's Certification, of Form I-693 (in an envelop sealed by the civil surgeon). Please see the instructions for Form I-693 for further information.

It is a medical condition that would need follow up or monitoring after arriving in the US. Some people get a waiver on a medical condition but have to prove treatment is given in the US. From the Center for Disease Control:

Class A conditions include:

  • active or infectious tuberculosis
  • untreated syphilis
  • untreated chancroid
  • untreated gonorrhea
  • untreated granuloma inguinale
  • untreated lymphogranuloma venereum
  • human immunodeficiency virus
  • Hansens disease
  • addiction or abuse of specific substance without harmful behavior and/or any physical or mental disorder with harmful behavior or history of such behavior, along with likelihood that behavior will recur.

Class B conditions include:

  • inactive or noninfectious tuberculosis
  • treated syphilis
  • other sexually transmitted diseases
  • pregnancy
  • treated, tuberculoid, borderline or paucibacillary Hansen’s disease
  • sustained, full remission of abuse of specific substances* and/or any physical or mental disorder (excluding addiction or abuse of specific substances but including other substance-related disorders) without harmful behavior or history of such behavior considered unlikely to recur (*amphetamines, cannabis, cocaine, hallucinogens, inhalants, opioids, phencyclidines, sedative-hypnotics and anxiolytics).
