Have you used Dr Arnold for vaccination translation recently?

Quick question for those who have used Dr Arnold as a way of translating your vaccinations onto the I-693 form recently.

I am sending him my vaccination records, 2 copies of page I-693 ( BUT DO I SIGN AND DATE THEM? or just sign them) they did tell me when i first called a month ago- but time has passed with getting married etc and ive forgotten and I cant get through to the clinic as its closed. I also cant remember the cost ( I am pretty sure its $30) but can anyone remind me as I'd like to get this sent off today.

Yes, I did. And I'm certain you sign and date them. Hopefully someone will back me up as I'm out at the moment and can't check the copies we sent

I'm certain about the $30 (was less when we did it) - can't say about the signature though. They're probably already closed today (it's 5:30 here) so you can't call and ask either.

I wonder if I understood that correctly: if I send I-693 to dr.Arnold, I should fill out part 1, sign and date the form, and the doctor fills out part 2 (only partially-the part about vaccinations) and part 5. what about the rest of part 2?

just sign and date, no need to fill anything else, the doctor will help you fill out as needed. very simple, sign and date, make a copy of your vaccination record and mail it...

I just went through DR. Arnold 2 weeks ago. You print the page that requires your signature and date it. On a seperate piece of paper write down all of your personnal info that the application asks for. I read somewhere that they do not like people just sending the papers in without calling them first, so i suggest you do that first. When you call the secretary will pick up, and just ask them if they are the office that handles immigration paper work. She will get your name and number and the nurse will call you back either the same day or the next. The nurse will tell you everything that you need. Good Luck!
