Hi everyone, i have a question on I-134. My fiance and I are preparing the docs this early. My fiance is Pinoy and has a second name (a filipino thing) and I just found out on his 1040 and W2 that his second name became his middle name...and his real middle name (his moms maiden name) was omitted In short, his name on these forms were different from his passports and other records. Would this be a problem? Is there anyone here had the same dilemma? thanks in advance for the reply =)

It depends. On his G-325A, did you list all the possible combinations of his name as they appear on his official documents?

04-05-2018: Case status updated - Interview Scheduled on May 10, 2018, 10:15 a.m. :D

05-10-2018: Citizenship Interview - Passed English and Civics Tests, Recommended for Approval! :D 

06-19-2018: Received email and text notification: Naturalization Ceremony Scheduled; waited for letter to be uploaded on online account - it has been set on Wednesday, July 25, 3:00 p.m.

No =( he didn't tell me. I hope this will not be much of a problem. Are they really particular on the 1040s and W2s? I hope not.

Could you give me an idea what you put on the forms? No need to give his real name...just tell me how his name appears on his passport, his birth certificate, and his tax documents.

For example...if the name that appears on his birth certificate is Juan Miguel Reyes Mercado...how does it appear on his other official documents?

it shouldn't be a problem for as long as in all other documents the second name is included, meaning in some documents his name is shorter (only the second name is included) and in some it's longer (includes the mother's maiden name)

but, yes, in your G325 you have to write down ALL the names he goes by

ex. on his 1040 : juan miguel dela cruz

on passport: juan miguel santos dela cruz

birth cert: juan miguel santos dela cruz

when you change your name later, you will find out that here in the US the middle name is the second name (whereas in the Phils middle name is mother's maiden name) and they don't use the mother's maiden name. so if you have only one name (ex john) and your last name is smith, and you don't have a middle name, then your legal name is john smith)

but you also have the option to include your mother's maiden name as your middle name... :)

His filipino name is (just an example) JUAN MIGUEL CRUZ REYES, whereas, CRUZ is his mom's maiden name. This name appears on his passport and birth certificate. On his 1040's, his name became JUAN M REYES, his second name MIGUEL became his middle initial/name. But on his pay stubs and other docs, he uses his complete name. Would this be a problem during the interview? What actions can i do now? Should i ask my fiance for a letter or something from the lawyer or from the tax office? Thanks so much for the reply. Sounds like you have it covered naman. The Feds will figure out the M is Miguel.
