Hello, I plan to use my passport as evidence of U.S. citizenship. However, for question 10 on the I-129F, it requires to "Give number of certificate, date and place it was issued." in regards to the birth certificate. Is the birth certificate number and place of issue required or should I write in the passport # or just leave that field blank? Also, "14. Address in the United States where your fiancé(e) intends to live." Right now I'm going to school and living in the dorms (graduate school). However, I will be finished and December and should have a job. Should I write in my parents address or?....

If you're a citizen by birth - leave those fields blank - as you see in the Example I'-129f in the Example forms section.

Also, "14. Address in the United States where your fiancé(e) intends to live."

I'd just put your parent's address - no problem.
