Hello all!

Please help us....we are filling out the application for change of status (permanent resident) and we need to visit a civil surgeon to sign off on my husband's vaccinations. The only problem is we don't know where this information is located...the doctor down in Chile gave him a sealed envelope and told him to give that envelope (without opening it) to immigration when he entered the United States. Would you suggest he get new vaccinations here and then have the civil surgeon sign off on it or who would he contact to get this information. We sent an email to the embassy down in Chile asking this same question. If they gave him the sealed envelope and told him not to open it how is he supposed to have this information for the I-693 form?!?!

I believe your husband should have been given 2 envelopes by the doctor in Chile: a chest x-ray and another that contained vaccination information (among other things) for your records. Perhaps they do things differently in Chile though (someone more familiar with the process over there should chime in and confirm or deny this, please). If you don't have that record, I believe you will need to contact the doctor and have him mail or fax you the vaccination records so you have them for the civil surgeon.

When the visa was issued, they gave him another envelope that was for immigration only, I suspect you are talking about that one. Didn't the doctor give him any documents to take home?
