filling out forms right now, need some help!!

I am filling out some forms right now for my fiancee's upcoming interview this month. There are a few questions where i am stuck and need your advices. Thank you very much in advance

Q1----on form ds-156k, question 5

My fiancee and I have a son together and he is a US citizen which currently over in Vietnam living with his mother. Should i choose WILL ACCOMPANY YOU or WILL FOLLOW YOU?? i am thinking accompany, what do you think

Q2-----on form ds-156k, does it need to be notarized? does my fiancee have to sign it before the Consular Officer?

Q3----on form ds-156 question 22, WHEN DO YOU INTEND TO ARRIVE IN THE US? WE HAVE NO IDEA WHEN, AND THE ONLINE FORM ONLY HAVE THE DATE OUTPUT, AND IT DOESNT LET YOU TYPE, SINCE WE DONT KNOW WHEN WILL THE BE THE EXACT DATE, LEAVE IT BLANK THEN? WHAT DID YOU GUYS PUT FOR YOURS?

Q4-----on form ds-156 question 34, NAMES and RELATIONSHIPS OF PERSONS TRAVELING WITH YOU?

our son who is a US citizen will be traveling with her to the US once she receives her visa, my mother will go back to Vietnam to take them over here, so what should i put for this question???

Have you looked at the example forms herer on VJ? Thery can be a big help.

Q1 - accompany

Q2 - As the instructions say, it is signed in front of the CO. She should not sign it beforehand. This form is not notarized.

Q3 - Blank

Q4 - list your son
