packet 4 officially lost in mail

I called DoS today and had a really nice agent answer. Turns out my packet 4 was mailed 2 weeks ago and I have not recieved it yet, so she told me consider it officially lost and notify Montreal ASAP as my interview is in less than 5 weeks.

I sent Montreal an e-mail saying that I have not recieved it yet and to resend the packet,....

BUT, I was hoping that someone who has recently gone through the interview or has recieved packet 4, could tell me what is in it other than the interview letter. I know that there is a post that people have written down what they have brought to the interview, but I haven't seen specific reference to the contents of packet 4.

I know I will feel better being able to at least prepare the forms and info that they request in the packet, while I wait for that elusive letter to arrive in the mail.

Thanks for the link Trailmix, but when I checked it out all I see are the contents of packet 3, which I have already. I'm just curious about packet 4 specifically, as I think, but am unsure of having to do the ds-156, ds-156k, and letter of intent, etc..

If anyone does have a packet 4 and could post me the contents it would be a great help and stress reliever.

I think it is just the letter confirming your interview date.

For Montreal the packet 4 just has your name, case number and interview date with a time that you can just ignore. It also says to make sure you bring all the documents they asked for in the packet 3, along with the DS 156 (don't forget the DS 156k either). It also gives you a list of prohibited items (no electronics, large bags etc.) and tells you who you can and cannot bring to the interview with you (you can bring your USC, a family member if you're disabled, no lawyers etc.). Nothing you didn't already know! Try your hardest to get Montreal to mail you a new one, because its your ticket in the door!

I have heard of people who printed out an email from MTL confirming their interview date and got in with that.
