USCIS Doctors nin Arizona

Hello,can someone tell me how to get a names and phone numbers of USCIS doctors in Arizona?Am waiting to have my vaccinations transcript for AOS final documents.Thanks.

Thanks buddy,called several offices and finally made an appointment for next monday.What bothers me thugh is the amount he is charging.The y told me to bring 60dollars.Is that okay or is the amount too high?

I booked it anyways since its the only document keeping me from mailing my package.

Do they give me the transcript copy the same day or by mail?

I think $25-$30 is about as low as it has been reported
