This may be answered somewhere and I have just missed it.

I will be filing a K-1 visa for a mother with 11 year old daughter. is there a form for the daughter's visa or is she atomically included in her mother I-129f form?

This site is very good. I'm happy to find it and thankful to all that take the time to help others.

There is no separate form or fee but you must make sure she is named on the I129-F that you'd fill out for her mother. Later once they are in the USA and you marry you would file them both separately to adjust their status.

On the I-129f petition you will list ALL children who are single and under age 21. At the time of applying for the visa(s) also known as the "interview" you will have the opportunity to apply for a K-2 visa for the child. This is a separate application based on the same petition. The daughter will have to have a medical exam and file all the same applications as your fiancee, pay the same fees, and go through the AOS process exactly as your fiancee. She will not need a police certificate under age 16, and does not have to attend the interview if under age 14. You will most likely (depending on country) need some form of permission from the father of the child OR a court order of sole custody by the mother. The daughter will also have to have a separate I-134 by you, the sponsor.

Nothing is automatic in this process. You MUST list her in the petition and you MUST apply for a visa when the opportunity comes. The petition is NOT a visa application, it is a petition to request that a foreign person be allowed to apply for a visa under the rules for a fiancee.

There is no separate petition form or petition fee. You do not need to complete a G-325a for the child.

There is most definitely separate visa applications, medical exams, affidavits of support, AOS and all associated fees for a separate visa and AOS. They must be completed, submitted, and paid for, or the child will not receive a visa.
