Do you think it will cause a probem if the Affidavit has only signature and date, without the full name written? If it matters the form is the new one (that doesn't need notarization )

I apologize if the question is a bit silly but I think I am freaking out. A lot....

Presumably you are referring to the bottom/signature portion of the form - in which case you do not need to write your name (just sign/date). :thumbs:
