I LOST MY WALLET AND MY GREEN CARD WAS INSIDE IT, NOW I WANT TO APPLY FOR A NEW ONE BUT I DONT UNDERSTAND SOME QUESTIONS IN 3RD PART.

FIRST QUESTION: City of Residence where you applied for an Immigrant Visa or Adjustment of Status

SECOND QUESTION: Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted

THIRD QUESTION: City/Town/Village of Birth

FOURTH QUESTION: Date of Admission as an immigrant or Adjustment of Status

FIFTH QUESTION: Destination in U.S. at time of Admission

SIXTH QUESTION: Port of Entry where Admitted to U.S.

THESE ARE THE QUESTIONS AND ALSO I LOST MY SOCIAL SECURITY CARD TOO AND DONT REMEMBER THE NUMBER BUT THIS FORM IS ASKING ME FOR IT...WHAT DO I DO PLEASE????????? I NEED HELP PEOPLE FOR GOD SAKEEEEEEEEE

What don't you understand about the questions? They seem pretty straight forward?

About your SSN - did you work at all? It should be on your W-2. Perhaps on a drivers license?

Perhaps some of your old paperwork?

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.

kinda seems a lot of strange questions to me too. Most are very self explanatory.

NVC sent packet to Manila May 12 2008

Interview set for June 18

Visa Denial, Contested & Approved

PDOS June 23

Flight to USA June 26

Arrival at DTW Visa accepted

Welcome to America!!!

Married 07/18/2008

Applied SS 07/22/09

Applied AOS around same time

Very happily married

EAD received Sept.

AP received Sept

No RFE's ...??

AOS Appointment 04-02-2009

AOS approved! Green card to be sent 3-6 weeks

1) what city did you live in when you applied for AOS

2) which consulate gave you the greencard, probably the USCIS office like vermont service center or california service center, it depeds on where you lived, look it up

3)Where were you born

4) What date did you come into the U.S

5) not sure, maybe where you were going when you were admitted in the U.S

6) where did you first enter the U.S like my husband's port of entry for his k-1 visa was el paso texas,

as for your social security card, maybe you should call uscis and ask if they have it on record, maybe you should call them and speak to someone and explain the situation.

Next time i wouldnt recommend keeping that stuff on you. Copies, ok, the real stuff, no, it belongs in a safe and secure spot. My husband got robbed in mexico and thank god he didnt have anything too important, just some money, drivers license, my pictures, that kinda stuff, but if your worried about immmigrtion or w/ecarry copies, im sure yolu can always call a loved 1 to go to your house and get the stuff out to bring it to you i they dont believe you. GOOD LUCK.

March 10, 2009 AOS paperwork sent, will wait for it to arrive via tracking #

March 12, 2009 AOS paperwork arrives in Chicago

March 23, 2009 NOA1 for I-485, I-131, I-765

March 30, 2009-Biometrics letter received, appt date April 10th @11am philly office

April 10, 2009-Biometrics appt. done, in and out in 15 minutes

Nov. 18, 2009-Card production ordered

Thanks for writing back...I arrived in the states in January and all this is really new to me...Nope i never worked before, i just got my SSN in about 2 weeks and the mistake i made is i kept it in the wallet with the green card and my country driver's license which i dont think it will be useful here...my wife is the one that filled for me and did all the paper work but she has been sick, she got lung cancer and i dont see her helping for now because she cant talk or think of anything for now but sleepy everyday in hopsital and thats what brought me down here....so the problem i am having with the qustions is i dont understand the english, i think that is the easy way to say it...If you can explain them to me, i will be grateful.

The rest of your numbers and info should be in your folder regarding immigration, if not photo copies originals.

I already checked the Envelope and they didnt write the # there but only on the SS Card. She didnt apply for SSN when she applied for me visa, I had to apply for it myself and do you think they will give me another one and what are the documents i need to take with me?

Thanks alot for this information....but i am having one more problem and its the first question, I didnt apply it but my wife filled for me, what city should i write? back in my country or here in the states?

I would think putting where YOU were at the time of application for the first question. That's what I put there. Remember, some of those questions don't need to be answered if there is no asterix and I'm sure there's the choice of putting 'not known' where the visa was issued.

Your SS, why not just go to your nearest SS office and take your passport. That is what I did though they won't issue a card until they see my drivers license which I don't have yet as it was in my wallet that was stolen. They did though give me proof of having a SS number. Not 100% on that one if you don't know your number...

As for carrying your green card, it states to always carry it with you or face a penalty. The idea of someone going to your house to get it is a good one though it depends where you are and whether or not they would accept that as you are caught not carrying it.
