Vaccination Supplement

Hi there,

Has anyone done the vaccination supplement form recently? I'm just wondering if I can send the vaccination record that I have from my initial medical along with my AOS forms. I've looked at the guides for AOS here, but the I-485 instructions itself does not specifically say this. Has anyone done this? Any RFEs?

Most of the civil surgeons I called wanted to do a full medical on me even though I do not need it. I remember from a while ago there were some posts about a civil surgeon in Texas that will do the forms via fax/mail and didn't charge a ridiculous amount of money. I do not remember the doctors name. Does anyone know what I'm talking about?

It is required, simply visit a Civil Surgeon with form I-693 IN-Hand, and highlight the paragraph in the instructions that explains that you only need vaccination part done.

The receptionist you are talking to on the phone is clueless about K-Visas, you need to visit the office and clearly explain that you just want the vaccinations record done.
