i'm on the process of preparing all the necessary documents for my AOS. For those who are from Philippines and just recently submitted there documents on AOS, do we need a civil surgeon for the signature of I-693? :unsure: i had my TD vaccine here in department of health, which the last thing i need to comply with medical and vaccine requirements. there is an instructions in I-693 that if we are K-1 Visa holder and already had a medical exam in our country (St. Lukes) we are not required to have another medical exam as long as AOS is filed within one year of our medical examination (St. Lukes). but what confused me is " A designated civil surgeon must complete the vaccination section". how will a civil surgeon complete the vaccination section when i already had all the vaccines i need???... :unsure::blush:

03/24/2009 - checked cashed by USCIS (day 7)

03/24/2009 - NOA1 for AOS & EAD notice date

03/26/2009 - received hardcopy of NOA1 for AOS and EAD (day 9)

03/26/2009 - Biometrics appointment notice date for AOS and EAD

03/30/2009 - received hardcopy of Biometrics appt. date for AOS and EAD (day 13)

04/13/2009 - Biometrics schedule for AOS and EAD (day 27)

04/13/2009 - Biometrics Done

05/11/2009 - EAD Card Production Ordered (day 55)

05/14/2009 - EAD Card Production Ordered Again (day 58)

05/18/2009 - email from CRIS EAD Approval Notice Sent (day 62)

05/18/2009 - EAD Approval Notice received - card attached (day 62)

05/20/2009 - applied SSN and State ID

05/29/2009 - received from mail SSN

05/30/2009 - received from mail state ID

08/17/2009 - Department of Homeland in Reno interview schedule

thank God, it was so easy interview and it took us 5mins then i was approved.

08/24/2009 - got my temporary residence card.

You take a copy of your DS 3025 and a record of the shots you received elsewhere [Health Dep]. The civil surgeon will transfer everything to I-693. LOTS of civil surgeons will try and hit you up for the full medical. Don't let them do so.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

thank you so much for the information, it is very helpfull. do the department of health where i got the shots need to fill-up I-693 form number 6?. do the surgeon need to seal I-693 before he gave it to me?.thanks again...

Civil surgeon will transfer the record from the health department. They'll seal the I-693 also. Get a copy.
