Hi VJ members, my wife and I had been in search of a Civil Surgeon for our upcoming AOS interview, couldn't find any listed in the phonebook but found it here on this webpage http://uscis.gov/graphics/exec/cs/Index.asp?EmptyForm, hope it will help.

we got ours last year, it turned out there was a huge difference in price of one from the other, so list your local office and ask folks if they have dealt with one in your area. AS for us we got a bit lucky, our doctor had just gone through the similar experience a few months before when she had to do this for her mother. She gave us the names of civil surgeons who turned out are the cheapest in the area. the whole thing took $60 to just fill out the paperwork, we already had the vacc done with our doctor paid by the health insurance.

so bottomline just call up all of the listed civil surgoens and compare prices before setting on one....

Definitely call around, you will find a wide variance in fees, at least we did.
