Never received DS 3025. What now?

Hey i am new to this AOS Thread so i hope someone can help me out.

I almost got all the documentation together to send in the AOS package. The only problem i got it that i never received the DS 3025 at my medical in Frankfurt. Has anyone ever had this problem before?

11/16/01 - Met during my stay as an exchange student

04/29/08 - Got engaged

05/02/08 - Send off I-129 package to VSC via USPS

05/05/08 - I-129 package delivered at VSC

05/15/08 - Check cashed

05/19/08 - Received NOA 1

06/24/08 - Touched

06/25/08 - Touched

09/04/08 - NOA2

09/11/08 - NVC send file to Frankfurt

09/16/08 - Frankfurt received my file

09/19/08 - Received Pack 3

09/30/08 - Returned Pack 3

10/17/08 - Received Pack 4

10/27/08 - Medical

10/27/08 - Interview

11/06/08 - Received my Visa in the mail

11/21/08 - One way ticket to ATL :)

12/30/08 - Wedding!!!

AOS Timeline

01/12/08 - Send off AOS package with EAD and AP

01/14/08 - Received by Chicago office

01/22/09 - NOA1 hardcopies in the mail

My husband didn't receive his DS 3025 either. He later phoned the doctor's office and requested that they mail it to his parents. His Mom received it in a few days and then when she came here for our wedding, she brought it with her.

04-08-2008 - Medical Exam

04-17-2008 - Interview!!!

04-22-2008 - VISA IN HAND!

05-02-2008 - U.S. Entrance

06-14-2008 - MARRIAGE!

Phase 2....

06-19-2008 - Sent AOS/EAD/AP

06-26-2008 - AOS/EAD/AP NOA1

07-19-2008 - Biometrics

07-22-2008 - Transferred to CSC!

07-28-2008 - Email from CRIS confirming CSC received case

08-25-2008 - Received AP in the mail (NO STATUS CHANGE OR CRIS EMAIL)

08-29-2008 - Received EAD card in the mail (NO STATUS CHANGE OR CRIS EMAIL)

09-22-2008 - Received Email from CRIS for I-485 "CARD PRODUCTION ORDERED!"

09-27-2008 - Received Green Card in the mail!

Phase 3....

06-29-2010 - Sent I-751 and package to Remove Conditions

07-06-2010 - NOA1

Also, your timeline says you are getting married April 4, 2009. I assume you just mean the ceremonial wedding then, right? You must be legally married within 90 days of entering the States on a K1 Visa...
