Great that you checked out the Wiki page! Yes, they're the same. I don't know if it's listed there yet, we might have to update it..but there is a new form you have to fill out now since October 22, 2008 and that's the DS-160 form.

Good point Carla, i'll post a link to that on the Wiki today.

Here it is skinny:

Important notice

Beginning October 22, 2008, all applicants applying for nonimmigrant visas in Montreal and Vancouver must use the new DS-160 application form, which is accessible on https://ceac.state.gov/genniv/.

After completing the new DS-160, the applicant will electronically submit the application and will be provided with a confirmation page that includes only limited biographical information and a bar code. Applicants must bring this confirmation page with them to their interview. Please note: if the online application has not been completed fully and properly the applicants case cannot be processed and a new interview appointment will be required.

Also this thread discusses the 'passport book' number you will be asked for:

New requirement, DS-160, passport book number?

This thread might also be helpful, in the last couple of pages they talk about what things you need to get together before starting to fill out the DS-160:
