Questions on form I-765 (Employment Authorization)

Hey guys, (yea i know you guys are bored of me, sorry)

i have questions on document I-765 Application for Employment Authorization.

On question 12 of the document it asks “Date of Last Entry into the U.S.” my husband has entered only once, so I guess that would be the date, correct? Question #13 asks the “Place of Last Entry into the U.S.” would that be the city? Or airport? And the most important question #15 “Current Immigration Status” what would my husband be considered? He came on a K1 visa. We got married before the 90 days. But the 90 days have passed already and we still haven’t filed for AOS because we were still gathering the funds (didn’t think our wedding was going to be so expensive lol) his 90 days were up on November 11. today is December 10, 2008, so what would his status be today? Thanks guys.

p.s. I know some of these questions are out the loop easy but I just want to be sure. Thanks again
