Hi,

I've just received my AOS interview date (January 13th 2009) and I'm very pleased!

I had a question regarding my vaccination record and the I-693.

My info: came into the US from England and married a US citizen on a K-1. I had an MMR and need another in about 6 months. The others were either deemed 'not appropriate' because of my age or VH (varicella history).

I handed over my 'sealed envelope' at the POE (they didn't ask to see my x-ray, which I still have) leaving me with a vaccination record from Knightsbridge Doctors. I submitted by AOS package with the vaccination record (not the I-693) and received no RFE. My appointment letter arrived today. My questions are:

1. On my appointment letter it states bring your I-693 and vaccination record (if not already sent). I sent my vaccination record with my AOS package, but not an I-693 (I don't have one - that information presumably reached them via the sealed envelope I gave at the POE. I've read that K-1 visa people don't need the I-693, only the vaccination record. Is this true?

Do I need to bring the FIRST part of the I-693 (or just the vaccination record, or neither? I have 'already sent' the vaccination record, after all)?

If so, do I fill it out myself or have a doctor do it? IF a doctor needs to be the one to do it, do I need another medical exam?

2. I have my vaccination record from Knightsbridge. Does this need to be transcribed onto the vaccination portion (the SECOND part) of the I-693 and then placed in a sealed envelope by a doctor's office, or not?

3. Many of the vaccinations required were not applicable to me, marked as such on the form, and the box requesting a waiver was checked. I received no varicella shot because Varicella History (VH) was marked on my form.

Is this alright, or do I need more shots?

4. USCIS ruled that after August, things like Zoster and human papillomavirus were required. These were on the doctor's form completed during my medical in June, and were deemed inappropriate then. Is that correct, or do I NOW need them?

Thanks, and sorry for reposting familiar content, but I've found contradictions elsewhere.

We've commented on this a bazillion times, but we don't know the answer. We are dying for you to tell us what happens at your interview and guide the rest of us with June 1 or later medicals who didn't get the tuberculin skin test, etc. and didn't do an I-693. Or did have their immunizations transcribed onto the new I-693, but didn't do the TST.

To be safe you should take all your shot records and information to the CS and let them evaluate if you need any additional vaccines and TB skin test or chest x-ray if needed. The CS will give you a sealed envelope containing the I-693 when completed.
