which form to fill out at SS office?

Timeline

have upcoming Oath ceremony and as everyone does, planning to stop by at the SS office on my way back. I know once becoming citizen, we have to notify the office, is there any particular form that we have to fill out? or just hand the certificate and SS card to be endorsed?

N-400:

04-21 - Sent to TSC

04-24 - Payment cashed

04-28 - NOA and biometric letters sent

05-02 - NOA and Biometric letters rcvd

05-13 - biometric appointment

08-18 - Interview scheduled (Memphis)

11-21 - Oath Scheduled - Done with USCIS!

11-21 - applied for passport - awaiting....

Any changes I have had to make required the SS-5. Seems like every time I go in they have me fill one out. Don't know if that helps any.

That's correct. You have to fill out SS-5 form, and marked your status as US Citizen.

You are supposed to fill out a form? I went in to our local office, handed my passport and SS cards over as well as my son's and asked that we be upgraded to Citizens as well as got the certain line taken off our old cards. The woman at the office took my information, punched it into the computer, then said we would receive our new cards in about 10 days.

thanks all for responding. I am going to take a filled out the SS-5 form with me but won't give it to them unless they ask. that way we can confirm if a form is indeed required.

I did that 2 days ago.I asked the clerk " should I fillout a form to chnage my status"and she is like "off course".So you have to fill out the form and you will get a replacement card in the mail in 2 weeks with same number,they will just update your status.

Jul 24,09 Pending,you can call every day for update but the day they call him for passport it means something is up so you get to know it before we do

Aug 06,09 Call from USEM requesting to submit passport

Aug 07,09 Every thing looks very good,visa issuance is up to the consulate!!!

Aug 07,09 Passport submitted

Aug 07,09 USEM is requesting applicant's passport :)

Aug 17,09-call 1 Visa issued Aug 3rd,EM recvd passport on 10th but still havent returned it

Aug 17,09-call 2 Case pending some kind of processing (URGHHHHHHHHH NOT AGAIN)

Aug 18,09 call to pick up passport,picked up from AMEX

Sep 26,09 REUNION :)

Usually they have same kind of screen as SS-5 form does.

So technically you can talk to officer over the window, and just go over each field for Question and answer session.

Then they can print out the receipt and give it to you.

But sometimes it takes more time, and possibly some typo.

So I prefer to fill out the form SS-5, and pass it to the officer.
