DS-3025 is similar to the I-693 Vaccination Supplement that you read about. It contains the same information but in a different format.

The Panel Physician who performs the visa medical exam will have all of the necessary forms. What rebeccajo is recommending is that Gavin should ask the Panel Physician for a photocopy of the completed DS-3025 at his exam. Then later on, when he applies for adjustment of status, he can include the DS-3025 with his I-485 in lieu of the I-693 Vaccination Supplement. If all vaccinations are either complete or waived, the DS-3025 may be accepted as sufficient.

Just make sure he gets the DS-3025. You will need it for his AOS.

Is that the vaccination supplement form I have heard about?

I don't know what the BCG is, (sounds like a booster you had in school at about 14 years old) but I do know that he needs Tetanus & Diptheria, not just Tetanus.

i just received my vaccination/immunization history from my doctor. i have had the mumps & measles done and i also have a bunch of "DPT'S," "OPV's" and a "DT." i believe that these are abbrevitions, does anybody know what they are?
