Legally Change of first name for form G325a

hi im new to this forum... i have some few questions in in this forum and i hope u can help me please...

1. im from the philippines and my fiance' is from US i have a question in filing up G325a form i never been married before but i legally changed my first name 2 years ago and im 29 right now ... should i put my old name in "all other names used" or should i state n/a or none .

2. applicants residence for the past 5 yrs?---i live in the city and we dont have a province or state here i wonder if i just put none or n/a.
